Online Forms

Dear Patients,

To help the Practice adapt to recent GP contractual changes, we will be changing our online form provider from Monday 24th November 2025.

This update will slightly change the online patient journey. The new questionnaire is more in-depth and follows NICE guidelines, helping us deliver a more accurate and efficient triage process.

We would also like to highlight the following:

• Creating a profile: The new system may require you to create a profile with Anima. Please be reassured that your medical data is not stored on this platform, and it is completely safe to use.

• More effective consultations: The detailed questions allow clinicians to gather key information before triage or consultation. This means your appointment time can be better spent discussing your concerns rather than collecting preliminary details.

• Support with the system: If you experience any difficulties using Anima, please contact the Practice by phone and let us know. We are here to help.

We appreciate your patience during this transition and hope the new system improves your experience with submitting enquiries.
